Appointments

If you need to see a doctor or nurse you should make an appointment by either contacting us online via our website or by ringing the surgery. If the matter is medically urgent and you do not have an appointment we will always arrange for you to be seen.

If you are unable to keep an appointment, please let us know as soon as possible, so we can offer your appointment to another patient. We require at least half an hours notice when cancelling an appointment.  

Nursing Services

Our Practice Nurses are Rachel Lathwood and Ellena Syrett . Rachel offers an Outreach clinic to patients every Tuesday. Our Nurse services include Adult and Children Immunisation, Cervical Smears, Hypertension Reviews and General Nursing services Our nurses are available Monday-Friday.

Same-Day Clinic   

The Same-Day Clinic offers same day access to a health professional for patients who request to be seen urgently with a minor illness when a doctor’s appointment is not available.  The Same-Day Clinic is offered by our Doctors and Clinical practitioners.  

Please be aware, for patients attending the surgery to be seen we offer a BUSY same day clinic. Therefore, there are times when there could be a longer wait for patients to be seen.

Routine Pre-Bookable Appointments

Health Care assistant, Denise, Rachel and Practice Nurse; Rachel offer routine pre bookable appointments which include blood pressure monitoring, dressings, health promotion and NHS health checks ages 40-74, and reviews. 

Home Visits

If possible please try to telephone reception before 11:00am if you require a home visit.

A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.

House visits are only available for patients who are housebound because of illness or disability.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre.

Sickness Certificates

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.

Evidence that you are sick

If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).

It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise.

You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.

Statement of Fitness for Work - ’Fit Note'

The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.
